CLIENT OVERVIEW

For more than half a century, North Kansas City Hospital (BKCH) has been the Northland’s preferred healthcare provider. The hospital serves the community with 451 licensed beds, more than 3,000 employees, and 508 physicians representing 46 medical specialties. From a Level III neonatal intensive care unit to home health and hospice services, NKCH provides a lifetime of care. Through its Meritas Health subsidiary, North Kansas City Hospital offers the largest network of physician practices in the Northland, with more than 140 physicians and 40 advanced practitioners in 36 convenient locations.

The Challenge

  • Retain steerage of health plan dollars and utilization when medically appropriate
  • Develop Population Health management programs to improve the health status of the workforce and their dependents, over 6,600 total covered lives
  • Implement a patient-centered medical home shared-savings initiative
  • Quantify impact of interventions and proactively develop strategies for benefits

APH Action Plan

Using Advanced Plan for Health (APH) data analytics risk engine, Poindexter, custom built for population health management, assess NKCH’s healthcare costs, identify opportunities and map out an actionable path to provide a cost-effective, self-funded benefits plan based on increased collaboration and shared risk. 

  • Stabilize health plan costs
  • Aggregate and provide data insights, stratify health risks of the population
  • Target areas for improvement in preventive medicine screening and medicine compliance
  • Develop wellness program with on-site wellness coordinator: health risk assessment, biometric testing, and individual health coaching
  • Implement disease management program for chronic conditions
  • Monthly team meeting to review performance and develop strategies for improvement

Results

  • 85% of employees complete an annual health risk assessment
  • 60% of employees fully participate in wellness program
  • 80% of health plan's adult enrollees utilize an integrated system primary care physician
  • While maintaining stable benefits, integrated health system's collaborative health plan strategy has contributed to a cumulative savings of over $200 million in the past 12 years
